3. Your contact information: Full legal name, mailing address, telephone number, and email.
4. Statement of good faith: That you believe the use is not authorized by the copyright owner.
5. Statement of accuracy: That the information is accurate, under penalty of perjury.
6. Signature: Your physical or electronic signature.

Counter-Notification

If you believe that material you posted was removed by mistake, you may file a counter-notification containing:

1. Your physical or electronic signature
2. Identification of the material that was removed
3. A statement under penalty of perjury that the material was removed by mistake
4. Your name, address, and consent to jurisdiction

Repeat Infringers

Y2Mate maintains a policy of terminating access for users who are repeat infringers.
